You are hereby notified that a Petition for Guardianship has been filed in this matter before the Sequoyah County District Court. A hearing on said Petition is set for the 6th day of December 2007 at 1:30 P.M. before the Honorable A.J. Henshaw Jr., Judge of the District Court to determine whether guardianship should be granted in the matter.

YOU ARE THEREFORE NOTIFIED THAT THE COURT WILL HEAR EVIDENCE IN SUPPORT OF AND IN OPPOSITION TO THE GRANTING OF GUARDIANSHIP AT THE PLACE AND TIME LISTED ABOVE, INCLUDING THE RIGHT TO OBJECT TO THE GUARDIANSHIP OF THE MINOR CHILD. YOUR FAILURE TO APPEAR AT SAID HEARING SHALL CONSTITUTE A DENIAL OF YOUR INTEREST IN THE CHILD, WHICH DENIAL MAY RESULT, WITHOUT FURTHER NOTICE TO YOU OF THIS PROCEEDING. IF YOU WOULD LIKE TO HAVE AN ATTORNEY REPRESENT YOU AT SAID HEARING BUT CANNOT AFFORD ONE, AN ATTORNEY MAY BE PROVIDED FOR YOU.
