To enroll a child, the parent must bring an original birth certificate, updated shot record, Social Security Card and CDIB card if applicable. Also proof of residence (electric bill, water bill, etc.) The child must be potty trained. Before Sept. 1, Pre-K3 must be age 3, Pre-K4 must be age 4 and Kindergarten must be age 5.
All documents must be present at the time of enrollment or your child’s enrollment cannot be completed.
Hearing and vision screenings will not be conducted, so students do not have to be present during pre-enrollment. The state of Oklahoma requires the following shots before a child may enter school.
Pre-K students must have: four DTaP, three Polio, one MMR, three Hep B, two Hep A and one Varicella. Kindergarten students must have: five DTaT, four Polio, two MMR, three Hep B, two Hep A, and one Varicella.
